Hair shaft thickness directly affects visual coverage. Thicker hair may cover a larger surface area than very fine hair, even when two patients receive a similar number of grafts.
Other factors that are also considered include:
- Hair texture.
- Degree of curl or wave.
- Contrast between hair color and scalp color.
- The proportion of follicular units containing one, two, or more hairs.
- The presence of miniaturized hairs.
Hair density and shaft diameter both influence how full the donor area appears after extraction.
Follicle Stability
Not all visible hair on the back or sides of the scalp necessarily belongs to a stable donor area.
When signs of miniaturization, diffuse hair loss, or retrograde alopecia are present, some follicles may become thinner in the future. Extracting hair from an unstable area may affect the longevity of the result and the appearance of the donor region.
For this reason, it is important to examine and document any miniaturization before surgery and to consider the hair loss pattern the patient may develop over time.
What Happens If the Donor Area Is Weak?
Having a donor area with limited density does not automatically mean that a hair transplant is impossible. It means that the treatment plan must be especially strategic and that expectations should be adjusted to the patient’s actual donor capacity.
Depending on the evaluation, it may be necessary to:
- Reduce the number of extracted follicles.
- Prioritize the frontal hairline over other areas.

5. Protect Overall Hair Health
When medically appropriate, treatment for active hair loss may help preserve existing hair and stabilize further thinning.
Medications or complementary procedures should only be recommended after reviewing the diagnosis, medical history, and potential contraindications. Not every treatment is suitable for every patient.
